Dear ESBA Labs,

 

I have practiced hair transplantation for 15 years and have performed thousands of procedures on

men and women, young and old and many races.

 

I have found the topical administration (premedication) of TOPICAINE® 4% Gel to be a useful adjunct

to help alleviate the discomfort associated with SQ injection of local anesthetic (2% lidocaine with

1:100,000 epinephrine).

 

The area to be premedicated is a band of skin approximately 1.5cm wide starting about 1cm superior

and 1cm anterior to the top of the ear and traversing across the upper forehead to the other ear with

the upper border of the band ending about 1cm below the patient's hairline.

 

After noting the patient's allergies and skin condition, the skin is prepped by rubbing the area with

alcohol pads as per manufacturer's instructions. A small amount of 4% gel is rubbed into the prepped

area starting about one hour prior to SQ injection of the local anesthetic. Every 10 to 15 minutes

additional small amounts of 4% gel are rubbed into the same area. A total of 10 grams of gel are used

over the hour.

 

1st time patients are asked to assess relative discomfort levels between the premedicated (with 4% gel) fronto-temporal areas versus the non-premedicated (without 4% gel) parieto-occipital areas upon

subsequent SQ injection of local anesthetic.

 

Repeat patients are also asked to assess the above as well as any change in discomfort level from a

previous procedure.

 

In conclusion most patients report a decrease in pain associated with the SQ injection of local anesthetic when premedicated with TOPICAINE® 4% Gel. Even patients with needle phobia or a low pain threshold seem to derive benefit.

Case Report: TOPICAINE® 4% useful for wound debridement

 

 

Philadelphia, November 2001

 

 

A 39 year old African/American nurse underwent a hypertrophic scar revision on back of her hand (dimensions: 2.5/7.5/0.25 cm); significant hypergranulation tissue formed; requiring extensive coldknife debridement. The fresh easily bleeding hypersensitive site was prepared as follows: a phlebotomy tournequet was placed at the wrist; TOPICAINE® Gel 4% was thickly applied to the site for 12 minutes; and a 1 to a million Klein tumescent epinephrine soaked compress was applied with pressure for 6 minutes which allowed a leveling shave resection done with a Gillette double edged razor blade. The patient reported no discomfort, and this allowed the procedure to proceed without any waste of time. Bleeding was well controlled and an Oasis wound dressing was applied and wetted with sterile saline; a fluff gauze and Kling secured the dressing and the patient left surgery in a satisfactory condition.

it barely over a month, I can give you a satisfactory report due to the volume of patients Dr Hopping sees on a regular basis which has used TOPICAINE® Gel. The effectiveness of TOPICAINE® Gel surpasses both EMLA and Ela-Max (now LMX). Patients have commented that they feel the area numbs faster and they really like the clear gel consistency. The price of TOPICAINE® Gel is much less than the other two products.
